Evaluations have focused on a fairly narrow range of clinical conditions (in particular diabetes, asthma and arthritis) and a limited range of clinical settings. The evidence is mixed, but studies of disease-specific educational programmes have tended to produce more positive results than generic approaches (Hampson 2001; Gibson et al. 2003; NICE 2003; Riemsma 2003; Wolf 2003). While self-management programmes play an important role in encouraging self-care, other initiatives also broadly support greater patient involvement in the process of care.

Although health literacy does not directly correlate with other inequalities in health, low health literacy appears to be particularly prevalent among lower socioeconomic groups, the elderly, ethnic minority groups and those with chronic medical conditions or disabilities (Andrus and Roth 2002; Sihota and Lennard 2004). Inadequate health literacy can have profound health and financial consequences, leading to impaired ability to self-care, poorer health status, higher rates of hospital admission, lower rates of adherence to treatments, less use of preventive services and weaker understanding of health-promoting behaviours.
